Job summary

Right at Home West Palm Beach is seeking a highly organized Care Manager with a background in social work and/or RN to oversee care for seniors in a home-care setting in West Palm Beach and nearby territories, including Sebastian, Port St. Lucie and Fort Pierce.

You will coordinate in-home evaluations, develop individualized care plans, supervise care staff, monitor service delivery, and act as liaison among clients, families and healthcare providers to ensure high-quality, person-centered care.

Qualifications

• Ideal Candidate: Minimum of 5 years of experience in home care, healthcare, or social services
• Knowledge of home care regulations, healthcare practices, and community resources
• Pass criminal background check (Level 2 AHCA)
• Excellent interpersonal, communication, and organizational skills
• Ability to assess client needs and develop appropriate care plans
• Leadership experience supervising staff
• Compassionate, patient, and dedicated to improving clients' quality of life
• Education: Social Work or related field, Bachelor’s degree in Nursing, LPN License
• Certification: CPR

This role is ideal for a professional who understands the complexities of care transitions, discharge planning, and ongoing support to older adults and their families. The Care Manager is to oversee and coordinate care services for our clients in a home care setting. The Care Manager will serve as the primary point of contact for clients and their families, ensuring personalized, high-quality care that promotes independence and well-being. Additional territories include Sebastian, Port St Lucie and Fort Pierce.
