Job summary

Right at Home San Luis Obispo seeks a Client Care Liaison who can oversee the client experience, work with clients, families, caregivers, and professionals to ensure high-quality care. The role emphasizes relationship-building, problem-solving, and staying organized in a fast-paced home-care setting.

You will assess needs, update care plans, and monitor service delivery while maintaining accurate records and coordinating with referral partners and office staff.

Responsibilities

• Oversee the client experience from start of care to ongoing relationship.
• Coordinate with clients, families, caregivers, office staff, referral partners, and professionals.
• Develop, implement, and update individualized care plans.
• Maintain regular, proactive communication to ensure services meet needs.
• Monitor care quality and address concerns as they arise.
• Identify changes in client condition and relay to appropriate team members.
• Support caregiver matching and mentor caregivers for safe, high-quality care.
• Maintain accurate client records and documentation.
• Help clients identify community resources to support continuity of care.
• Build relationships with referral partners and professionals in the community.
• Represent Right at Home professionally at events and meetings.
• Collaborate with the office team to solve problems and improve client experience.
