Job summary

Right at Home West Palm Beach is seeking a highly organized Care Manager with a background in social work and/or RN to oversee care for seniors in a home-care setting in West Palm Beach and nearby territories, including Sebastian, Port St. Lucie and Fort Pierce.

You will coordinate in-home evaluations, develop individualized care plans, supervise care staff, monitor service delivery, and act as liaison among clients, families and healthcare providers to ensure high-quality, person-centered care.

Qualifications

• Ideal Candidate: Minimum of 5 years of experience in home care, healthcare, or social services
• Knowledge of home care regulations, healthcare practices, and community resources
• Pass criminal background check (Level 2 AHCA)
• Excellent interpersonal, communication, and organizational skills
• Ability to assess client needs and develop appropriate care plans
• Leadership experience supervising staff
• Compassionate, patient, and dedicated to improving clients' quality of life
• Education: Social Work or related field, Bachelor’s degree in Nursing, LPN License
• Certification: CPR

Responsibilities

• Access clients' needs through in-home evaluations and collaborate with families and healthcare providers
• Develop and implement individualized care plans tailored to each client's preferences and medical requirements
• Assist in training and supervising care staff, including caregivers, home health aides and certified nursing assistants
• Monitor service delivery to ensure quality standards are met and adjust care plans as needed
• Act as a liaison between clients, families, healthcare professionals, third party provider partners and internal teams
• Maintain accurate documentation, including care plans, progress notes and compliance records
• Ensure compliance with all relevant regulations, licensing, and safety standards
• Provide ongoing support and education to clients and their families regarding care options and resources
