Job summary

Right At Home Sacramento-Davis seeks a Care Coordinator to design personalized home-care plans and coordinate with physicians, families, and caregivers. You’ll ensure adherence to plans, monitor progress, and train staff to deliver high-quality care.

Ideal candidates have 1–2 years of related experience, HIPAA knowledge, and proficiency with scheduling software. This on-site role emphasizes compassion, organization, and clear communication to support clients and families.

Qualifications

Responsibilities

• Coordinate holistic care with physicians, families, and caregivers.
• Monitor daily services and progress toward goals; retrain staff as needed.
• Lead client advocacy by accompanying clients to appointments and relaying updates.
• Recruit and train caregivers; assist HR with onboarding and education.
• Build and adjust caregiver schedules for 30–40 clients.
• Document assessments and progress in EMR/CRM; generate quality reports.
• Resolve urgent issues with calm, confidence, and compassion.

What You’ll Do

• Coordinate holistic care: collaborate with physicians, families, and caregivers to craft individualized Care Plans that meet medical, emotional, and social needs.
• Ensure plan adherence: monitor daily services, review progress toward goals, and retrain staff when updates are needed.
• Lead client advocacy: accompany clients to physician appointments or hospital visits; relay updates to the healthcare team and family members.
• Recruit & train caregivers: assist HR in hiring, onboarding, and educating field staff on best practices and agency protocols.
• Schedule like a pro: build and adjust caregiver schedules for a caseload of ~30–40 clients, balancing continuity of care and employee preferences.
• Track outcomes: document assessments, incidents, and goal progress in our EMR/CRM; generate reports that drive quality improvement.
• Problem‑solve fast: resolve urgent issues with calm, confidence, and compassion—24/7 on‑call rotation shared with team [if applicable].

What You’ll Bring

• Certification / license: CMA, CNA, LPN, RN, or Social Worker credentials welcome (preferred but not required)
• 1–2 years’ experience as a Care Coordinator, Client Care Manager, Case Manager, or similar role—home‑care or healthcare setting highly valued
• Strong assessment, care‑planning, and documentation skills; familiarity with HIPAA and state regulations
• Proficiency with Microsoft Office and EMR/CRM scheduling software
• Excellent verbal and written communication; you build trust quickly with clients and clinicians alike
• Stellar organization: juggle multiple priorities, meet deadlines, and adapt in a fast‑paced environment
• Reliable transportation for client visits; valid driver’s license
