Job summary

Apploi is seeking a proactive Client Care Coordinator to own the client experience from intake through ongoing case coordination. This on-site role involves coordinating intake, onboarding, and caregiver scheduling while maintaining detailed case notes and escalating issues as needed.

You will lead cross-functional coordination with Scheduling, HR, and operations, ensuring timely updates, quality care, and client satisfaction across multiple counties during home visits.

Qualifications

• Experience in intake, family communication and active case follow-up.
• New Jersey-licensed LPN with coordination skills; supervision rules apply.
• Current nursing student with relevant healthcare, caregiving or care coordination experience.

Responsibilities

• Lead new client intake and care launch, coordinating intake visits and onboarding.
• Manage active caseload with proactive client and family communication.
• Coordinate caregiver orientation, training and field support.
• Monitor quality, document issues and coordinate timely resolutions.
• Ensure cross-functional coordination and accountability across Scheduling and HR.
