Job summary

Safe Harbor Home Care in San Diego is seeking a Client Care Coordinator to help lead our caregiving team. You will oversee client assessments, care plans, and matching caregivers to client needs while ensuring continuity of care.

This role emphasizes documentation accuracy, compliance with policies, and strong communication with clients, families, and staff. We value growth, training, and teamwork, with weekly pay and supportive benefits.

Qualifications

• Must be at least 18 years of age.
• California Home Care Aide (HCA) registration prior to client assignment.
• Current TB clearance.
• Valid driver’s license and reliable transportation.
• Ability to maintain accurate documentation and client confidentiality.
• Ability to pass required background check.
• Strong organizational and prioritization skills.
• Strong written and verbal communication skills.
• Leadership, coaching, and problem-solving abilities.
• Ability to manage multiple priorities in a fast-paced environment.
• Commitment to client satisfaction and quality care.

Responsibilities

• Conduct client assessments and reassessments.
• Develop and update client care plans.
• Match caregivers with client needs and preferences.
• Provide caregiving coverage when necessary.
• Train, mentor, and support caregivers.
• Coach and support caregiver development.
• Partner with Scheduling and Operations teams to maintain continuity of care.
• Monitor service quality and client satisfaction.
• Address client concerns and assist with service recovery.
• Support caregiver accountability and performance improvement.
• Maintain accurate client documentation and records.
• Monitor compliance with company policies and regulatory requirements.
• Communicate effectively with clients, families, caregivers, and office staff.
• Assist with onboarding activities and caregiver support initiatives.
• Support quality assurance efforts and follow-up activities.
