Safe Harbor Home Care is seeking a Client Care Coordinator in California to lead client assessments, develop care plans, and match caregivers to client needs. You will collaborate with Scheduling and Operations to ensure continuity of care and high client satisfaction, while guiding caregiver development and maintaining proper documentation.

The role emphasizes leadership, organizational skills, and strong communication as you support a growing team in a fast-paced home care environment.

Qualifications

• Must be at least 18 years of age.
• California Home Care Aide (HCA) registration prior to client assignment
• Current TB clearance
• Valid driver’s license and reliable transportation
• Ability to maintain accurate documentation and client confidentiality
• Ability to pass required background check

Responsabilités

• Conduct client assessments and reassessments
• Develop and update client care plans
• Match caregivers with client needs and preferences
• Provide caregiving coverage when necessary
• Train, mentor, and support caregivers
• Coach and support caregiver development
• Partner with Scheduling and Operations teams to maintain continuity of care
• Monitor service quality and client satisfaction
• Address client concerns and assist with service recovery
• Support caregiver accountability and performance improvement
• Maintain accurate client documentation and records
• Monitor compliance with company policies and regulatory requirements
• Communicate effectively with clients, families, caregivers, and office staff
• Assist with onboarding activities and caregiver support initiatives
• Support quality assurance efforts and follow-up activities
• Most Importantly - be someone our clients and caregivers can count on!

What You'll Do

• California Home Care Aide (HCA) registration prior to client assignment
• Current TB clearance
• Valid driver's license and reliable transportation
• Ability to maintain accurate documentation and client confidentiality
• Ability to pass required background check
• Strong organizational and prioritization skills
• Strong written and verbal communication skills
• Leadership, coaching, and problem-solving abilities
• Ability to manage multiple priorities in a fast-paced environment
• Commitment to client satisfaction and quality care

Preferred Qualifications

• Previous experience conducting client assessments
• Prior caregiving leadership experience
• CNA Certification
• HHA Certification
• Experience with WellSky or similar software
• Client relations, service coordination, or case management experience
• Bilingual (Spanish/English or Tagalog/English)
