Option Care Health is seeking a Practice Liaison, Chronic Therapies to improve referral-to-start conversion and manage provider relationships across chronic therapy programs. You will serve as the main liaison between providers, patients, and internal teams, using data and proactive account management to enhance access and patient experience.

You will work primarily remotely with up to ~20% travel, collaborating with Sales, Intake, Pharmacy, and Operations to resolve barriers and drive growth in

Qualifications

• High school diploma or equivalent is required.
• Bachelor’s degree in Business, Healthcare, Life Sciences, or related field preferred.
• Experience building relationships and driving measurable outcomes.

Responsibilities

• Build and maintain strong relationships with physicians, clinics, and referral sources.
• Assess referral workflows and identify barriers to timely patient starts and improve timelines.
• Partner with Sales, Intake, Pharmacy, and Operations to resolve issues and expand referral opportunities.
• Track account performance, referral trends, and growth opportunities using CRM systems.
• Guide patients and providers through onboarding and access questions and represent the organization at events.
• Collaborate cross-functionally with Sales, Operations, Pharmacy, and Intake to support referral flow.
