Suvida Healthcare LLC is seeking a Care Coordinator to work closely with the Nurse Care Manager in delivering high-quality care to high-risk patients in Garland, TX. Responsibilities include daily follow-ups, processing DME and home health orders, triaging calls, and retrieving hospital records for recently admitted patients.

The role requires Medical Assistant or LPN/LVN background, completion of a MA program, and a national certification (AAMA/CCMA or equivalent); bilingual English/Spanish is

Formación

• Minimum 3 years of Medical Assistant or LPN/LVN experience
• Completion of a Medical Assistant program required
• Certification in Medical Assisting from AAMA, CCMA or any other nationally recognized body required

Responsabilidades

• Conduct daily patient follow-ups for high-risk patients
• Process DME and home health orders
• Receive inbound triage calls and coordinate responses
• Retrieve hospital records for recently admitted patients
• Support Chronic Care and Transition of Care programs
• Assist in procedures within the Medical Assistant scope of practice
• Collaborate with the Nurse Care Manager to ensure seamless patient care
• Identify barriers to care for high-risk patients
• Coordinate care progression from acute to home or transitional facilities
• Communicate with physicians, nurses, and care team
• Facilitate discharge planning to optimize outcomes
• Monitor patient progress and intervene as needed
• Coordinate with external case managers and community resources
• Participate in clinical performance improvement activities
• Support Medicare HEDIS metric closure efforts
