Job summary

Suvida Healthcare is seeking a Care Coordinator to deliver high-quality care for Medicare-eligible Hispanic seniors through a multidisciplinary primary care program. The role emphasizes daily patient follow-ups, DME processing, triage, and records retrieval in a high-risk patient population.

Ideal candidates have 3+ years as Medical Assistant or LPN/LVN, with 5+ years preferred, and bilingual English/Spanish abilities.

Qualifications

• Minimum 3 years of experience as a Medical Assistant or LPN/LVN.
• 5 years’ experience as a Medical Assistant preferred.
• Experience in chronic care management preferred.
• Excellent interpersonal and communication skills.
• Bilingual English/Spanish required.

Responsibilities

• Conduct daily patient follow-ups for high-risk patients.
• Process DME and home health orders.
• Receive inbound triage calls and coordinate responses.
• Retrieve hospital records for recently admitted patients.
• Collaborate with Nurse Care Manager to ensure seamless care.
• Support Medicare HEDIS metrics and care gaps closure.
