Suvida Healthcare in Dallas, TX is hiring a Nurse Care Manager to join our multidisciplinary team and coordinate care for high‑risk seniors within our primary care program. You will collaborate with care teams to plan effective, patient‑centered care.

This role focuses on managing chronic conditions, overseeing transitions of care, addressing social determinants of health, and ensuring efficient resource use. Bilingual English/Spanish is preferred; Texas RN license required.

Qualifications

• Minimum 2 years of RN experience in care management or similar.
• Experience with transitions of care and chronic disease management is desirable.
• Bilingual in English/Spanish preferred.

Responsibilities

• Coordinate care for high‑risk patients within the care team.
• Oversee transitions of care from acute to post-acute settings.
• Address social determinants of health to improve patient outcomes.
• Collaborate with physicians, nurses, payers, and community resources to ensure high-quality, cost-effective care.
