Job summary

Washburn County is seeking a dedicated Service Facilitator to support children in the CLTS and CCS programs. Under supervision, you will assess needs, arrange services, and monitor outcomes for families, helping children remain in home and community.

This role emphasizes collaboration with families, providers, and a multi-disciplinary team, with a focus on recovery concepts and respectful service delivery.

Qualifications

• Bachelor's degree in Social Work or related Human Services degree.
• Previous experience in providing mental health or CLTS case management services preferred.
• Must have access to private transportation and a valid State of Wisconsin driver’s license.
• Ability to work independently and as part of a team.

Responsibilities

• Assists children and families identify, secure and monitor appropriate services.
• Collaborates with a multi-disciplinary team to develop and update service plans.
• Provides crisis and short-term intervention as needed.
• Maintains confidential records and timely documentation.
• Participates in meetings and ensures program requirements are met.
