Qualifications

• Bachelor's degree in social work or related field plus one year of experience.
• CLTS Waiver Certification required within 30 days of hire.
• Valid Driver's License is required.

Responsibilities

• Provides assessment, case planning, protection, and case management services in compliance with federal and state regulations.
• Develops and reviews individual service plans based on health, mental health, and family needs.
• Determines crisis risk and coordinates with other programs to resolve it.
• Refers clients to community resources and ongoing supports.
• Connects families to community services and promotes family involvement in community life.
