Lutheran Social Services of Wisconsin & Upper Michigan is hiring a Care Coordinator for the CLTS Waiver Program in Walworth County. This full-time role offers a hybrid work option and requires a degree in Social Work with experience serving children with disabilities.

You will coordinate services, support families, document diligently, and collaborate with agencies while maintaining regulatory timelines and quality care.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Minimum 1 year experience with children/youth with disabilities.
• Spanish bilingualism preferred but not required.
• Proficient in computer systems and electronic health records.
• Strong interpersonal and organizational skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess children’s functional abilities using approved tools.
• Develop and implement individualized service plans with families and providers.
• Coordinate team meetings and services based on family goals.
• Maintain accurate documentation and meet regulatory timelines.
• Collaborate with internal teams and external agencies.
• Testify in legal proceedings when required.
• Communicate clearly with families and team members.
• Participate in staff development, training, and supervision.
