Job summary

St. Francis Children’s Center is seeking a CLTS Service Coordinator to manage a caseload of 50–55 children, ensuring waiver services are delivered per policy.

The role requires bilingual skills in Spanish and strong coordination across internal and external teams, including participating in ISP development and required PAS/PFA documentation. A valid Wisconsin driver’s license and adherence to timelines are essential.

Qualifications

• Licensed or certified per Wis. Stat. § 457.08 and at least one year of experience with children/youth with disabilities.
• Must have a valid Wisconsin driver’s license with limitations on violations/accidents and auto liability coverage.
• Course of study or degree in health/human services with related experience acceptable per DHS waiver requirements.

Responsibilities

• Coordinates delivery of Children’s Long Term Support services across agency lines and serves as the single point of contact for families.
• Upholds waiver rules, regulations, and deadlines set by state and county.
• Attends meetings and trainings related to CLTS.
• Maintains a caseload of 50–55 clients with monthly service units.
• Develops, reviews, and updates ISP and required documentation (PAS, PFA, annual screens).
• Maintains regular contact with families and documents timely case notes within 48 hours.
• Assists with interpretation/translation as needed.
