Job summary

Alpine Physicians seeks a Care Manager (Social Worker) to deliver clinically informed, person-centered management for Medicaid members with complex needs. You will assess, plan, and coordinate care across interdisciplinary teams to improve outcomes and reduce avoidable utilization.

You will perform transitions of care, engage hard-to-reach members, and educate patients and families while maintaining HIPAA compliance and high-quality documentation.

Qualifications

• Active MSW with licensure in good standing.
• Licensed in the state where the population is served.
• 1+ year of care management, coordination, or related experience.

Responsibilities

• Perform comprehensive clinical and psychosocial assessments.
• Develop and update individualized care plans.
• Manage transitions of care after hospitalization or acute events.
• Coordinate with PCPs, specialists, and community resources.
• Educate patients and caregivers on disease management and navigation.
