Job summary

Alpine Physician Partners is seeking a Care Manager (Social Worker) to deliver clinically and psychosocially informed care management for Medicaid members with complex needs. You will perform assessments, develop care plans, and support care coordination across teams to improve engagement and outcomes.

This licensed professional will conduct transitions of care, engage hard-to-reach members, and ensure compliant documentation while aligning with HIPAA requirements.

Qualifications

• Active MSW license in good standing.
• Experience in care management, coordination, or transitional care.
• Experience with high-risk, medically complex populations preferred.

Responsibilities

• Perform comprehensive clinical and psychosocial assessments for high-risk members.
• Develop and update individualized care plans addressing medical and social needs.
• Manage transitions of care and follow-up within timelines.
• Coordinate with PCPs, behavioral health providers, and community resources.
• Deliver patient and caregiver education on disease management and navigation.
