Qualifikationen

• Active unrestricted RN license in good standing.
• Must be licensed in the state where the assigned population is served.
• 1+ years of experience in care management, care coordination, or related fields.

Aufgaben

• Perform comprehensive clinical and psychosocial assessments for high-risk, medically complex members.
• Develop and update individualized care plans addressing medical, behavioral health, and social needs.
• Manage transitions of care post-hospitalization or ED visits and other acute episodes.
• Coordinate care with PCPs, specialists, and community resources.
• Provide patient and caregiver education on disease management and care navigation.
• Maintain HIPAA-compliant documentation across all care activities.
