Job summary

Legion Health is building an AI-native care platform in San Francisco, combining doctors and AI to deliver fast, affordable care. We are hiring a principal-level engineer to own the most critical systems—across architecture, data, and agent orchestration—delivering scalable, auditable care with regulatory compliance and rapid velocity.

The role requires shipping at velocity, owning end-to-end problem spaces, and guiding a growing engineering discipline from design to documentation.

Qualifications

• Experience owning architecture across multiple codebases.
• Ship high-volume AI-agent driven systems with auditability and safety.
• HIPAA-compliant practices and fail-closed defaults are required.

Responsibilities

• Own the factory spine and scale PRs from 500–1,000 to 10,000 per month.
• Own the clinical work spine: event-sourced system of record for orders, tasks, and receipts.
• Manage agent operations in production with tool use, checks, and receipts.
