Job summary

Signature HealthCARE is seeking a caring LPN/LVN to join our Memphis facility. You will provide direct nursing care, administer medications, and support residents with activities of daily living while maintaining dignity and safety.

You’ll supervise nursing assistants, document care, and participate in care planning with an interdisciplinary team in a dynamic long‑term care setting. Ideal candidates hold an active LPN/LVN license in state, with 1+ year of clinical nursing experience, and

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory/charge nurse role preferred.
• Familiarity with EHR systems and MS Office.

Responsibilities

• Provide direct nursing care to residents including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physicians of changes in condition.
• Develop, implement and review individualized resident care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers and discharges.
• Participate in QA, staff education and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
