Job summary

Signature HealthCARE in Memphis, TN is seeking a dedicated LPN/LVN to join our caring team. You will provide direct nursing care, administer medications, and assist residents with ADLs.

The role includes supervising nursing assistants, maintaining accurate documentation, coordinating admissions and discharges, and contributing to quality assurance. A current LPN license, CPR/BLS, and at least one year in clinical nursing (long-term care preferred) are required or preferred.

Qualifications

• Active LPN/LVN license in the state of practice.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• CPR/BLS certification preferred.
• Familiarity with electronic health records (EHR) systems and MS Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor residents’ health status and notify physicians of changes.
• Develop and review individualized resident care plans with the team.
• Document accurately per facility policy and procedures.
