Job summary

Signature HealthCARE of Clarksville is seeking an dedicated LPN/LVN to deliver direct nursing care, manage medications, and support residents with daily living activities in a 120-bed facility. You will supervise nursing assistants and coordinate with the interdisciplinary team to maintain comfort, safety, and quality of life.

Qualifications

• Active LPN/LVN license in state of practice.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• CPR/BLS certification preferred.
• Experience in a supervisory/charge nurse role preferred.
• Familiarity with EHR systems and Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the shift.
• Monitor residents’ status and notify physicians/parties of changes.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance and staff education.
