Job summary

Signature HealthCARE in Memphis is seeking a dedicated LPN/LVN to provide direct nursing care and support the care team. You will manage medications, monitor residents, and contribute to care plan development in a dynamic long-term care setting.

Qualifications

• Current, active LPN/LVN license in the state of practice.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
