Job summary

Signature HealthCARE of Primacy in Memphis, TN is seeking a dedicated LPN/LVN to provide direct nursing care to residents, administer medications, and support daily living activities within a compassionate, team-based environment.

You will supervise nursing assistants, coordinate with the interdisciplinary team, document care accurately, and participate in quality assurance activities. A valid LPN/LVN license and CPR/BLS are preferred, with at least one year of clinical nursing experience.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Strong verbal and written communication skills.
• Ability to work independently and exercise sound clinical judgment.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
