Job summary

Community Bridges, Inc. is seeking a Peer Support Specialist to provide experience, strength, and hope to individuals and families navigating recovery and care transitions within a multidisciplinary team.

You will collaborate with RNs, PCTs, and BHCs to coordinate care, support discharge planning, and engage social supports in treatment planning. A high school diploma or GED is required; six months recovery from substance use or mental health disorders is needed.

Qualifications

• Highschool diploma or GED required; associate degree or higher preferred for advancement.
• Minimum six months of recovery from substance use or mental health disorders required.
• Health care related experience in crisis situations is a plus but not required.

Responsibilities

• Coordinate care with the treatment team and ensure involvement of family and social supports in discharge planning.
• Work with a multidisciplinary team (RN, PCT, BHC) and supervisory staff to support care transitions.

Education
