Job summary

Victorian Post Acute in San Francisco, CA, is seeking a PRN Occupational Therapist (OTR) to join our in-house therapy program at 2121 Pine Street. Flexible scheduling and a resident-centered care approach support your impact on daily independence and quality of life.

You will evaluate residents, develop treatment plans, supervise COTAs, and collaborate with physicians, nursing staff, and families to maximize outcomes. California OTR license is required; new graduates are encouraged to apply.

Responsibilities

  • Evaluate residents and develop individualized occupational therapy treatment plans.
  • Deliver skilled occupational therapy services per physician orders and plans of care.
  • Supervise COTAs in compliance with California regulations.
  • Collaborate with physicians, nursing staff and families to maximize outcomes.
  • Participate in interdisciplinary care conferences and discharge planning.
  • Complete timely documentation including evaluations, notes, progress reports, and discharge summaries.
  • Educate residents and caregivers on adaptive equipment and home programs to maximize independence.
  • Recommend and coordinate durable medical equipment as needed.
  • Maintain a safe, organized and efficient therapy environment.

Key Responsibilities
  • Evaluate residents and develop individualized occupational therapy treatment plans.
  • Deliver skilled occupational therapy services in accordance with physician orders and established plans of care.
  • Supervise Certified Occupational Therapy Assistants (COTAs) in compliance with California regulations.
  • Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to maximize outcomes.
  • Participate in interdisciplinary care conferences and discharge planning meetings.
  • Complete timely and accurate documentation, including evaluations, treatment notes, progress reports, recertifications, and discharge summaries.
  • Educate residents and caregivers on adaptive equipment, safety strategies, home programs, and techniques to maximize independence.
  • Recommend and coordinate durable medical equipment (DME) as needed.
  • Help maintain a safe, organized, and efficient therapy environment.
Qualifications
  • Current California Occupational Therapist (OTR) license in good standing.
