Job summary

PACS Group seeks a PRN Occupational Therapist (OTR) for Victorian Post Acute's in-house therapy program in San Francisco, CA. This per diem role offers flexible scheduling and a pathway to impactful patient care.

Responsibilities

  • Evaluate residents and develop individualized occupational therapy treatment plans.
  • Provide skilled occupational therapy services in accordance with the established plan of care.
  • Supervise COTAs in compliance with California practice guidelines.
  • Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to achieve optimal outcomes.
  • Participate in care conferences, rehabilitation meetings, and discharge planning.
  • Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
  • Educate residents, families, and caregivers on adaptive equipment, home programs, safety strategies, and functional independence techniques.
  • Recommend and coordinate appropriate DME to enhance resident safety and independence.

Victorian Post Acute is a beautiful 90-bed, rehabilitation-focused skilled nursing facility dedicated to providing outstanding care for both short-term rehabilitation and long-term care residents. We are currently seeking a compassionate and motivated Occupational Therapist (OTR) to join our growing in-house therapy team. Key Responsibilities
  • Evaluate residents and develop individualized occupational therapy treatment plans.
  • Provide skilled occupational therapy services in accordance with the established plan of care.
  • Supervise Certified Occupational Therapy Assistants (COTAs) in compliance with California practice guidelines.
  • Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to achieve optimal outcomes.
  • Participate in care conferences, rehabilitation meetings, and discharge planning.
  • Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
  • Educate residents, families, and caregivers on adaptive equipment, home programs, safety strategies, and functional independence techniques.
  • Recommend and coordinate appropriate durable medical equipment (DME) to enhance resident safety and independence.
  • Maintain a safe, organized therapy environment and promptly report equipment concerns.
Qualifications
  • Current California Occupational Therapist (OTR) license in good standing.
  • Strong communication, organizational, and interdisciplinary collaboration skills.
