Avamere Skilled Advisors, LLC d/b/a Avamere Living is hiring for a Certified Nursing Assistant (CNA) at Avamere Three Fountains in Medford, OR. This full-time, overnight NOC shift (10:00 PM to 6:00 AM) provides routine daily nursing care in accordance with each resident's assessment and care plan.

The role emphasizes compassionate caregiving, collaboration with nursing staff, and maintaining a warm, calm environment for residents' well-being and safety.

Qualifications

• Must have active Oregon CNA license.
• Must maintain CPR/BLS during employment.
• Must pass a background check and fit requirements.

Responsibilities

• Check residents' personal care needs per care plan and daily routines.
• Review care plans daily for changes and updates.
• Inform nurse supervisor of changes in resident condition.
• Assist residents with daily activities and transportation to appointments.
• Foster a warm, patient, cheerful environment and support residents' comfort.
• Ensure rooms are ready for receiving residents and assist as needed.
• Communicate resident information to charge nurses and administrators.

Certified Nursing Assistant CNA

Job Summary

The primary responsibility as a Certified Nursing Assistant is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan.

Essential Duties and Job Responsibilities

• Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.


• Review resident care plans daily to determine if changes in the resident's daily care


• routine have been made on the care plan.


• Inform the nurse supervisor of any changes in the resident's condition so that


• appropriate information can be entered on the resident's care plan.


• Assist residents with daily needs, including preparation for activity and social programs, and transporting residents to/from appointments.


• Create and uphold an atmosphere of warmth, patience, enthusiasm, and foster a calm and cheerful environment.


• Ensure residents' rooms are ready for receiving and help residents feel comfortable.


• Participate in facility surveys by authorized government agencies.


• Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator.

Requirements:

• Have a 10th grade education or above


• Be a licensed certified nursing assistant in this state


• Must maintain and have an active CPR/BLS during employment
