Job summary

Avamere is hiring a Certified Nursing Assistant (CNA) for the full-time day shift (6:00 AM to 2:00 PM) at Avamere Three Fountains, Medford, OR. The role requires providing routine nursing care per each resident’s assessment and care plan.

The ideal candidate will assist residents with daily needs, maintain a caring environment, and support care team communications. CPR/BLS active certification is required.

Qualifications

• High school diploma or equivalent is required.
• Licensed CNA in the state is required.
• Active CPR/BLS certification during employment is required.
• Passion for caregiving and improving residents' quality of life.

Responsibilities

• Check residents to ensure personal care needs are met as per wishes.
• Review care plans daily for changes.
• Inform nurse supervisor of changes for updating care plans.
• Assist with daily activities and transport to appointments.
• Create a warm, patient, cheerful environment for residents.
• Prepare rooms and help residents feel comfortable.
• Assist with facility surveys and regulatory compliance.
• Communicate essential resident information to nursing staff and administrator.

Certified Nursing Assistant CNA

Status: Full-time DAY Shift 6:00 AM to 2:00 PM

Location: Avamere Three Fountains: 835 Crater Lake Ave. Medford, OR

Job Summary

The primary responsibility as a Certified Nursing Assistant is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan.

Essential Duties and Job Responsibilities

• Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.


• Review resident care plans daily to determine if changes in the resident’s daily care


• routine have been made on the care plan.


• Inform the nurse supervisor of any changes in the resident’s condition so that


• appropriate information can be entered on the resident’s care plan.


• Assist residents with daily needs, including preparation for activity and social programs, and transporting residents to/from appointments.


• Create and uphold an atmosphere of warmth, patience, enthusiasm, and foster a calm and cheerful environment.


• Ensure residents’ rooms are ready for receiving and help residents feel comfortable.


• Participate in facility surveys by authorized government agencies.


• Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator.

Requirements and Qualifications

• Have a 10th grade education or above


• Be a licensed certified nursing assistant in this state


• Must maintain and have an active CPR/BLS during employment


• Have a passion for caregiving and enhancing the quality of life for our residents
