Avamere Three Fountains in Medford, OR is hiring a Certified Nursing Assistant (CNA) for the full-time night shift (10:00 PM to 6:00 AM). The role involves routine daily nursing care aligned with each resident's care plan and assessments.

You will assist with daily activities, help with mobility, and ensure a warm, calm environment for residents. Qualifications include a high school education or above, active CNA licensure in Oregon, and CPR/BLS certification.

Qualifications

• Have a 10th grade education or above.
• Be a licensed certified nursing assistant in this state.
• Must maintain and have an active CPR/BLS during employment.
• Have a passion for caregiving and enhancing the quality of life for our residents.

Responsibilities

• Check residents' personal care needs per their preferences and wishes.
• Review resident care plans daily to determine changes in the daily care routine.
• Inform the nurse supervisor of changes so data is updated in the care plan.
• Assist residents with daily needs, including activity preparation and transportation to appointments.
• Create a warm, patient, enthusiastic environment and foster a calm atmosphere.
• Ensure residents' rooms are ready for receiving and help them feel comfortable.
• Participate in facility surveys by authorized government agencies.
• Communicate necessary resident information to charge nurses, the director of nursing, and/or administrator.

Job Summary

The primary responsibility as a Certified Nursing Assistant is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan.

Essential Duties and Job Responsibilities

• Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
• Review resident care plans daily to determine if changes in the resident’s daily care
• routine have been made on the care plan.
• Inform the nurse supervisor of any changes in the resident’s condition so that
• appropriate information can be entered on the resident’s care plan.
• Assist residents with daily needs, including preparation for activity and social programs, and transporting residents to/from appointments.
• Create and uphold an atmosphere of warmth, patience, enthusiasm, and foster a calm and cheerful environment.
• Ensure residents’ rooms are ready for receiving and help residents feel comfortable.
• Participate in facility surveys by authorized government agencies.
• Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator.

Requirements and Qualifications

• Have a 10th grade education or above
