Avamere Living in Medford, OR is seeking a dedicated Certified Nursing Assistant (CNA) for the Full-time evening shift (2:00 PM–10:00 PM) at Avamere Three Fountains, 835 Crater Lake Ave. You will provide routine daily nursing care in line with each resident's assessment and care plan.

As a CNA, you will support residents with daily needs, review care plans, communicate with nurses, and help maintain a warm, respectful environment.

Qualifications

• Must be a licensed certified nursing assistant (CNA) in Oregon.
• Active CPR/BLS certification required during employment.
• 10th grade education or above.

Responsibilities

• Assist residents with daily needs and activities.
• Review and follow resident care plans daily.
• Inform nurse supervisor of changes in resident condition.
• Communicate resident information to charge nurses and supervisors.
• Create a warm, patient, and cheerful environment.
• Ensure residents' rooms are prepared and comfortable.
• Support and transport residents to activities and appointments.
• Participate in facility surveys as required.
• Maintain accurate documentation per care plans.

Status: Full-time EVE Shift 2:00 PM to 10:00 PM

Location: Avamere Three Fountains: 835 Crater Lake Ave. Medford, OR

Job Summary

The primary responsibility as a Certified Nursing Assistant is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan.

Essential Duties and Job Responsibilities

• Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
• Review resident care plans daily to determine if changes in the resident's daily care
• routine have been made on the care plan.
• Inform the nurse supervisor of any changes in the resident's condition so that
• appropriate information can be entered on the resident's care plan.
• Assist residents with daily needs, including preparation for activity and social programs, and transporting residents to/from appointments.
• Create and uphold an atmosphere of warmth, patience, enthusiasm, and foster a calm and cheerful environment.
• Ensure residents' rooms are ready for receiving and help residents feel comfortable.
• Participate in facility surveys by authorized government agencies.
• Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator.

Requirements and Qualifications

• Have a 10th grade education or above
• Be a licensed certified nursing assistant in this state
• Must maintain and have an active CPR/BLS during employment
• Have a passion for caregiving and enhancing the quality of life for our residents
