Job summary

Moments Hospice is seeking a compassionate CNA to provide essential daily living assistance to patients, with support from a dedicated RN Case Manager. This role emphasizes patient comfort, dignity, and teamwork in a fast-paced home care setting.

Qualifications

• Must hold CNA license and provide 12 months caregiving experience.
• Valid driver's license and auto insurance.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.

Qualifications:

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)
• Excellent customer service skills
• Strong communication (written, verbal, and active listening)
• Problem-solving skills
• Time management skills
• Ability to work in an electronic medical record system
• Ability to lift 50 lbs.
• Good understanding of body mechanics
• Strong understanding of professional boundaries
• Valid driver's license (required)
• Valid Auto Insurance (required)
