Job summary

Moments Hospice is seeking a Certified Nursing Assistant to support patients on the road, delivering personal care and care coordination under an RN Case Manager. Expected duties include daily living assistance, care plan reviews, and EMR documentation.

Applicants must have a CNA license and at least 12 months of caregiving experience; CPR is a plus. A valid driver’s license and auto insurance are required to enable travel across the service area. Competitive pay and benefits are offered.

Qualifications

CNA license required.
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice required.
CPR certification preferred.
Excellent customer service skills.
Strong communication and active listening.
Ability to lift 50 lbs and use body mechanics.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
