Job summary

Moments Hospice is seeking a compassionate CNA to provide direct patient care, support daily living activities, and participate in care planning under an RN Case Manager. A company car and cell phone accompany a benefits package, with health, dental, and vision insurance and PTO.

Candidates must hold a CNA license, have 12 months of caregiving experience, and possess a valid driver's license and auto insurance. CPR certification is preferred, and EMR experience is a plus.

Qualifications

CNA license is required.
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
Current CPR certification preferred.
Valid driver's license required.
Valid Auto Insurance required.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition; Maintain electronic medical records.
Work under the direction of a Case Manager RN.
