Job summary

Moment s Hospice seeks a compassionate Certified Nursing Assistant to join our on-the-road care team, delivering essential daily living support under the guidance of an RN Case Manager. You will assist with personal care, mobility, nutrition, and care plan reviews in patients’ homes.

Qualifications

• Certified Nursing Assistant (CNA) license is required.
• 12 months of caregiving experience in home care or related settings.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong written and verbal communication skills.
• Good problem-solving and time management abilities.
• Ability to work in an electronic medical record system.
• Must be able to lift up to 50 lbs.
• Good understanding of body mechanics.
• Understanding of professional boundaries.
• Valid driver's license is required.
• Valid auto insurance is required.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
