Job summary

Moments Hospice is hiring a Certified Nursing Assistant to provide compassionate daily living support on the road. You will work under a Case Manager RN, assist with personal care, and maintain electronic medical records to ensure patient comfort and dignity.

The role includes a company car with fuel and insurance coverage, cell phone, and comprehensive benefits, with a competitive hourly rate and immediate PTO accrual.

Qualifications

• Certified Nursing Assistant (CNA) license required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required).
• Current CPR certification (preferred).
• Excellent customer service skills.
• Strong communication (written, verbal, and active listening).
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license (required).
• Valid Auto Insurance (required).

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range of motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN
