Job summary

Gardant is seeking a Part‑Time Certified Nursing Assistant at Heritage Woods of Dwight in Dwight, IL to deliver individualized resident care per each care plan. You will assist with activities of daily living, promote independence, and maintain dignity while communicating health updates to the clinical team.

Qualifications

• High School graduate or GED equivalent required.
• Current CNA certification.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver exceptional and individualized care per each resident's care plan.
• Assist with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate and impactful manner.
• Engage in learning opportunities through seminars, staff meetings, and other learning experiences.
• Comply with confidentiality and privacy rules for resident information.
• Perform any other duties as assigned.

• Deliver exceptional and individualized care as determined by each resident's care plan
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity
• Report changes in resident health status to the clinical team
• Connect with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences
• Comply with all rules and regulations regarding confidentiality and privacy of resident information
• Perform any other duties as assigned
