Job summary

Heritage Woods of Freeport is seeking a compassionate Certified Nursing Assistant for a part-time, 2nd shift position in Freeport, IL. You will provide individualized resident care per care plans and assist with ADLs while supporting independence and dignity.

Responsibilities include communicating health changes to the clinical team and engaging residents and families with empathy. Daily learning opportunities and adherence to privacy regulations are expected.

Qualifications

• High School graduate or GED equivalent required.
• Current CNA certification from an accredited program.
• Exceptional communication and interpersonal skills.
• Ability to serve with love, compassion and dignity.

Responsibilities

• Deliver exceptional, individualized care per resident care plans.
• Assist with Activities of Daily Living while promoting independence and dignity.

Salary Range: $18.00 - $18.50 Hourly

Job Shift: 2nd shift

Responsibilities

• Deliver exceptional and individualized care as determined by each resident's care plan
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity
• Report changes in resident health status to the clinical team
• Connect with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences
• Comply with all rules and regulations regarding confidentiality and privacy of resident information
• Perform any other duties as assigned
