Job summary

Grace Healthcare Services is seeking a Certified Home Health Aide (CHHA) to serve Bergen & Hudson Counties in New Jersey. The role provides direct patient care, including personal care, meal prep, and home management, with documentation using CellTrak and collaboration with RN Case Managers.

The position requires a valid NJ CHHA license, clear English communication, and the ability to lift up to 50 lbs. Weekend shifts and travel within the counties are included, with starting pay at $20 per hour

Responsibilities

• Provide direct care that makes a meaningful impact.
• Assist with personal care: bathing, grooming, dressing, skincare, mobility.
• Prepare simple, nutritious meals per care plan.
• Ensure a safe, clean, comfortable home environment.
• Document care and activities using CellTrak.
• Report condition changes to RN Case Managers for prompt action.

Weekends: 7;00am-3:30pm & every other weekend
Compensation: Starting at $20 per hour (based on experience)

What You'll Do

As a CHHA with Grace Healthcare, you'll provide direct care that makes a meaningful impact, including:

Personal Care: Assistance with bathing, grooming, dressing, skincare, and mobility

Meal Preparation: Creating simple, nutritious meals according to the patient's care plan

Home Management: Ensuring a safe, clean, and comfortable home environment

Documentation: Recording care and activities using CellTrak

Team Collaboration: Reporting condition changes to RN Case Managers for prompt action

What We're Looking For
