Grace Healthcare Services is seeking a Certified Home Health Aide (CHHA) for Middlesex, Union, and Somerset Counties, NJ. The role includes personal care, meal prep, home management, documentation using CellTrak, and collaboration with RN Case Managers to ensure timely care delivery.

The position offers hours Monday through Friday from 4:00pm to 8:00pm, with every other weekend 4:00pm–8:00pm, and starting pay at $20 per hour based on experience.

Formación

• Valid NJ CHHA license in good standing.
• Clear communication skills in English (reading, writing, and speaking)
• Ability to lift, push, or pull up to 50 lbs
• Dependable transportation and willingness to travel within Bergen & Hudson Counties

Responsabilidades

• Personal Care: Assistance with bathing, grooming, dressing, skincare, and mobility
• Meal Preparation: Creating simple, nutritious meals according to the patient’s care plan
• Home Management: Ensuring a safe, clean, and comfortable home environment
• Documentation: Recording care and activities using CellTrak
• Team Collaboration: Reporting condition changes to RN Case Managers for prompt action

Position: Certified Home Health Aide (CHHA)
Location: Middlesex, Union, and Somerset Counties, NJ

Monday - Friday 4:00pm-8:00pm & every other weekend: 4:00pm-8:00pm
Compensation: Starting at $20 per hour (based on experience)
