Job summary

Grace Healthcare Services in New Jersey is seeking a Certified Home Health Aide (CHHA) to provide compassionate in-home care in Bergen & Hudson counties. Weekends are required with shifts from 7:00am–3:30pm and every other weekend.

You will assist with personal care, meal prep, home management, and documentation, while coordinating with RN Case Managers to ensure quality patient outcomes. Starting pay is $20 per hour, based on experience.

Qualifications

• Must hold a valid NJ CHHA license in good standing.
• Clear communication skills in English (reading, writing, speaking).
• Ability to lift, push, or pull up to 50 lbs.
• Dependable transportation and willingness to travel within Bergen & Hudson Counties.

Responsibilities

• Personal Care: assistance with bathing, grooming, dressing, skincare, and mobility.
• Meal Preparation: creating simple, nutritious meals per care plan.
• Home Management: maintain safe, clean, comfortable home environment.
• Documentation: record care using CellTrak.
• Team Collaboration: report condition changes to RN Case Managers for prompt action.

What You'll Do
