Job summary

Grace Healthcare Services in New Jersey is seeking a Certified Home Health Aide (CHHA) to provide direct, compassionate care in Middlesex, Union, and Somerset Counties. You'll support patients with personal care, meal prep, home management, and documentation using CellTrak, reporting changes to RN Case Managers.

Require valid NJ CHHA license, excellent English communication, ability to lift up to 50 lbs, and reliable transportation with willingness to travel within the counties.

Position: Certified Home Health Aide (CHHA)
Location: Middlesex, Union, and Somerset Counties, NJ

Compensation: Starting at $20 per hour (based on experience)

What You'll Do

As a CHHA with Grace Healthcare, you'll provide direct care that makes a meaningful impact, including:

Personal Care: Assistance with bathing, grooming, dressing, skincare, and mobility

Meal Preparation: Creating simple, nutritious meals according to the patient's care plan

Home Management: Ensuring a safe, clean, and comfortable home environment

Documentation: Recording care and activities using CellTrak

Team Collaboration: Reporting condition changes to RN Case Managers for prompt action

What We're Looking For
