Job summary

Oak Street Health in the United States is seeking a Registered Nurse to join the Center Nurse team within a value‑based care model. You will support patient assessments, chronic condition management and collaborate with the care team to reduce avoidable hospitalizations.

You will perform skilled nursing procedures, provide education, conduct onsite testing and coordinate care with providers to ensure safe, high‑quality patient experiences.

Qualifications

• Active, unrestricted RN licensure in the state of residence is required.
• BLS or CPR certification is required.
• Proficient in EMR systems and able to document care accurately.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage.
• Perform skilled nursing procedures including wound care and vaccines.
• Provide patient and caregiver education on chronic conditions and preventative care.
• Coordinate with providers and care team to manage complex care plans.
• Review abnormal labs promptly to maintain safety and continuity of care.
