Oak Street Health is hiring a Center Nurse in Memphis to coordinate patient care and provide a seamless clinical experience. You will conduct assessments, deliver nursing care, and collaborate with the care team to implement care plans while maintaining patient-centered communication.

The role emphasizes data-informed care and creating a welcoming environment for patients and families. Strong RN licensure, BLS, and EMR experience are required, with opportunities to lead in a team-focused clinic

Competenze

• Active RN licensure in good standing with the applicable state.
• BLS Certification.
• EMR experience.
• Ability to maintain patient confidentiality and process information securely.
• US work authorization.
• Ability to assess patients without face-to-face interaction; strong communication and assessment skills.

Mansioni

• Provide competent nursing care safely and consistently.
• Respond to telephonic requests with dependability and responsiveness.
• Provide triage and symptom management remotely or in person.
• Utilize protocols for medication management, refills and prior authorizations.
• Review patient medications and update as needed.
• Educate and directly care for patients, especially around chronic conditions.
• Collaborate to monitor progress of care plans for patients on multiple provider panels.
• Form relationships with patients and caregivers to support prevention and ED/hospital diversion.
• Create a welcoming environment to meet patients and communities’ needs.
• Delegate activities to other clinical team members as needed.
• Participate in care team meetings to discuss patient care and clinic operations.
• Deliver an exceptional patient experience through service and respectful care.
• Perform point-of-care testing, procedures and phlebotomy as needed.
• Perform other related duties as assigned.
