Oak Street Health, part of CVS Health, seeks a Center Nurse in Memphis to deliver value-based care within a coordinated clinical team. You will perform assessments, chronic condition management, and patient education while coordinating with providers to prevent hospitalizations.

The position emphasizes protocol-driven care, documentation, and collaboration with the care team to ensure safe, high-quality patient outcomes for diverse communities.

Qualifications

• Active, Unrestricted Registered Nurse Licensure in the state you reside.
• Current BLS/CPR certification.
• Fluent written and spoken English language skills.
• US work authorization.
• Experience with Electronic Medical Records (EMR).
• Ability to maintain patient confidentiality.
• Ability to assess patients remotely with strong communication.

Responsibilities

• Support triage and assessment to determine appropriate care level.
• Perform wound care and medication administration (including vaccines).
• Educate patients and caregivers on chronic conditions and preventative care.
• Review medications and ensure safety and adherence during nurse-led visits.
• Conduct onsite testing (POC labs, EKGs, specimen collection) and review results.
• Collaborate with providers and the care team to manage complex care plans.
• Delegate tasks to medical assistants and phlebotomists.
• Ensure documentation and timely closure of patient visits.
• Provide emergency care per organizational procedures.
