Job summary

Oak Street Health in Memphis, TN is seeking a Center Nurse to join a value-based care team focused on preventive and coordinated patient care. You will perform assessments, administer vaccines, and manage chronic conditions in a collaborative clinic setting.

The role reports to the Practice Manager and emphasizes patient education, timely documentation, and coordination with interdisciplinary teams to improve health outcomes.

Qualifications

• Active, Unrestricted RN Licensure in the state you reside.
• BLS or CPR Certification required.
• Fluent written and spoken English language skills.
• US work authorization needed.
• Experience with Electronic Medical Records (EMR).
• Ability to keep patient information confidential.
• Ability to assess patients without face-to-face interaction and strong communication.

Responsibilities

• Support patient assessment and triage for walk-in and telephonic visits.
• Execute protocol-driven chronic condition management.
• Perform nursing procedures including wound care and vaccines.
• Educate patients and caregivers on chronic conditions and prevention.
• Review medications and ensure safety and adherence.
• Conduct onsite testing (POC labs, EKGs) and collect specimens.
• Collaborate with providers to manage complex care plans.
