Oak Street Health in Memphis, TN is seeking a Center Nurse to provide clinical care within a value-based model. You will conduct assessments, manage chronic conditions, and collaborate with the care team to prevent hospitalizations.

Responsibilities include wound care, vaccines, patient education, onsite testing, and care coordination with providers. 40 hours/week, full-time, with competitive CVS Health benefits.

Qualifications

• Active, unrestricted RN licensure in the residing state.
• BLS or CPR certification required.
• Fluent written and spoken English.
• US work authorization.
• Experience with Electronic Medical Records (EMR).
• Ability to maintain patient confidentiality and professionalism.
• Strong communication and patient assessment skills, including remote assessment.

Responsabilités

• Support patient assessment and triage to ensure appropriate level of care.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccines.
• Provide patient and caregiver education on chronic conditions and preventive care.
• Conduct medication reviews and ensure patient safety and adherence.
• Perform onsite testing (point-of-care labs, EKGs) and collect specimens.
• Collaborate with providers to coordinate complex care plans and transitions from hospital to clinic.
• Manage medication management protocols and prior authorizations.
