Job summary

Oak Street Health in Bronx, NY is seeking a Registered Nurse to deliver comprehensive clinical care within a value-based model. You will perform assessments, manage chronic conditions, and educate patients, collaborating with the care team to prevent hospitalizations and improve outcomes.

You will report to the Practice Manager and engage in triage, vaccine administration, onsite testing, and care coordination to ensure high-quality patient care and smooth clinic operations.

Qualifications

• Active, unrestricted RN licensure in the state you reside.
• BLS or CPR certification.
• Fluent written and spoken English language skills.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and process information confidentially.

Responsibilities

• Support patient assessment and triage for walk-in and telephonic visits.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccines.
• Provide education to patients and caregivers on chronic conditions and preventive care.
• Conduct medication reviews to ensure safety and adherence.
• Conduct onsite tests (point-of-care labs, EKGs) and use patient data to close gaps in preventative screenings.
• Coordinate care with providers and interdisciplinary team for hospital-to-clinic transitions.
• Utilize protocols for medication management, renewals and authorizations.
• Ensure timely clinical communications and close patient visit encounters.
• Delegate tasks to medical assistants and phlebotomists as needed.
