Job summary

Oak Street Health in Norristown, PA is seeking a Registered Nurse to support clinical assessments, chronic condition management and care coordination within a value-based care model.

You will perform wound care, vaccinations, medication reviews, and point-of-care testing while collaborating with a multidisciplinary team to reduce avoidable hospital visits and improve patient outcomes.

Qualifications

• Active, unrestricted RN or LPN licensure in the state of residence.
• BLS or CPR certification.
• Fluent written and spoken English.
• US work authorization.
• EMR experience.
• Ability to maintain patient confidentiality and process information securely.
• Ability to assess patients without face-to-face interaction.

Responsibilities

• Support patient assessment and triage to ensure appropriate care and avoid unnecessary visits.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccines.
• Conduct medication reviews during nurse-led visits for safety and adherence.
• Conduct onsite testing (labs, EKGs, specimen collection) and identify gaps in preventive care.
• Review high-priority lab results to maintain safety and continuity of care.
• Coordinate with providers and care team for complex care plans and weekly transitions from hospital to clinic.
• Utilize protocols for medication management, renewals, and prior authorizations.
• Ensure timely clinical communications and closure of patient encounters.
• Delegate tasks to medical assistants and phlebotomists to support flow.
• Perform other related duties as assigned.
