Job summary

Qualifications

• Active, unrestricted RN licensure in the state of residence.
• BLS/CPR certification.
• Fluent written and spoken English.
• US work authorization.
• Experience with Electronic Medical Records (EMR).
• Ability to maintain patient confidentiality and handle sensitive information.
• Ability to assess patients without in-person contact; strong communication and assessment skills.

Responsibilities

• Support nursing assessments and triage to ensure appropriate care and avoid unnecessary visits.
• Perform protocol-driven chronic condition management and vaccines.
• Conduct wound care and other skilled nursing procedures.
• Educate patients and caregivers on chronic conditions and preventive care.
• Review medications and undertake nurse-led visits for safety and adherence.
• Coordinate onsite testing (POC labs, EKGs, specimen collection) and analyze data to close care gaps.
• Collaborate with providers to manage complex care plans and weekly transitions from hospital to clinic.
• Utilize standardized protocols for medication management and prior authorizations.
